Routine Physical Exam
Purpose: To maintain health and catch problems early
Age: 18 & over
When: Annually or as directed by Health Care Provider
Eye Exam
Purpose: To check for problems including cataracts and glaucoma
Age: 20-39 baseline exam and if eye problems are suspected
40-64 every 2 - 4 years
65 & over Every 1-2 years
Dental Exam
Purpose: To check for cavities, early signs of gum disease and other
oral problems
Age: 18 & over
When: Every 6 months
The above exams should also include the following tests and
screenings consult with your health care provider for the appropriate
tests and schedules.
Blood Glucose
Purpose: Screen for Diabetes
Age: 45 years & over
When: Every 3 years or as directed by Healthcare Provider
Blood Pressure
Purpose: To detect high blood pressure or hypertension-a risk factor
for heart disease and strokes
Age: 18 & over
When: Every 1-2 years or as directed by Health care Provider
Cholesterol Test
Purpose: To check cholesterol content of blood if it is too high
a major factor in heart disease.
Age: 20 & over
When: Every 5 years or as directed by health care provider
Colorectal Exam
Purpose: To screen for colorectal cancer a highly treatable disease
if caught early
Age: 50 & over:
When:
- Fecal Occult Blood test every year
- Flexible Sigmoidoscopy every 5 years
- Fecal occult blood test every 5 years and
Flexible Sigmoidoscopy every 5 years
- Double-Contrast Barium Enema every 5 years
- Colonscopy every 10 years
Schedule Recommended by the American Cancer Society
Prostate Specific Antigen Test
Purpose: To detect prostate cancer
Age: 50 or older
When: Consult with your Health Care Provider
Testicular Exam
Purpose: Self-Exam to check for painless lumps that could indicate
testicular cancer
Age: 15 & over
When: Self Exam monthly and a professional exam when you have a
complete physical exam or upon early detection of lumps
Thyroid Screening
Purpose: To check for proper functioning of the thyroid gland
Age: 35 & over
When: Every 5 years or as directed by Health Care Provider
**If you have a family history or are at high
risk for certain diseases or conditions consult with your Health
Care Provider about the need for more frequent tests or screenings.
